Oral Conscious Sedation

Some children need more help than laughing gas can give. Maybe a previous visit went badly. Maybe they're very young and there's real work to do. Maybe the anxiety just runs high, and no amount of encouragement gets them through it.

Oral conscious sedation is our middle option — deeper than nitrous, lighter than full anesthesia. Your child takes medication by mouth, becomes calm and drowsy, and we complete the treatment in one comfortable visit.

When we recommend it

  • Significant dental anxiety, especially after a difficult past experience
  • Young children who need treatment they can't realistically hold still for
  • Several procedures that would otherwise stretch across multiple visits
  • Sensory or developmental needs that make a standard appointment overwhelming
  • A child for whom nitrous alone hasn't been enough

We don't suggest it lightly and we won't push it. If we bring it up, we'll explain exactly why we think it's right for your child — and then you decide.

Is your child a candidate?

Your child needs to be at least 3 years old and weigh at least 30 pounds for oral sedation in our office.

Beyond that, we review their full medical history together. Things we'll want to talk about include large tonsils or adenoids, snoring or sleep apnea, asthma or other breathing conditions, heart conditions, and any history of trouble with sedation or anesthesia. Some of these mean we'd choose a different approach; most just mean we plan more carefully.

Please tell us about everything your child takes — prescriptions, over-the-counter medicines, vitamins, supplements, herbal remedies. Some interact with sedation medication in ways that genuinely matter.

How it works

Your child takes the medication here at the office, and we wait with you while it takes effect.

As it comes on, they become sleepy and relaxed. Many doze. They aren't unconscious — they can still respond to us, and they breathe on their own throughout. Most children remember little or nothing of the appointment afterward, which is often the whole point.

We use a combination of midazolam, midazolam + hydroxyzine or meperidine +hydroxyzine, dosed to your child's weight and health history, usually alongside nitrous oxide. Dr. Mindi reviews the specific medications and doses with you at the consultation, before the day of treatment. You'll know exactly what your child is receiving and why.

Before the appointment

We'll call you the day before

Someone from our team will call to go through the preoperative instructions with you out loud, and we need to actually speak with you — not leave a voicemail. It's a short call, and it exists because these instructions are too important to leave to a page someone skimmed a week ago.

If we can't reach you, we may need to reschedule. Please make sure we have a number where you'll pick up.

Fasting: nothing to eat or drink for 8 hours

This is the single most important instruction on this page.

No food and no drinks — including water, milk, and juice — for the 8 hours before the appointment. For most families that means nothing after midnight the night before. No gum, no candy, no mints.

No brushing the morning of the procedure

Remove all temptations from bedrooms and car seats

Sedation is very safe on an empty stomach. When the stomach isn't empty, there's a risk that food or liquid comes back up and enters the lungs, which is a serious complication that may result in a hospital visit. That's the entire reason for the rule.

If your child eats or drinks anything inside that window, call us before you leave home. We will not be upset with you. We'd far rather reschedule than proceed when it isn't safe.

Everything else

  • Call us if your child is unwell. A cough, congestion, fever, or wheezing in the days before affects the airway. Let us know rather than hoping it clears — rescheduling is the right call, not an inconvenience.
  • Dress them in loose, comfortable clothes with short sleeves, so monitors go on easily.
  • Bring a comfort item. A blanket or stuffed animal helps more than you'd expect.
  • Bring a second adult if you can. One to drive, one to sit beside a drowsy child on the way home. It makes the trip much easier.
  • Clear the whole day. No school, no activities, no sports afterward.

How we keep your child safe

Someone from our team is with your child continuously, from the moment the medication is given until you leave together.

We monitor heart rate, blood oxygen, blood pressure, and breathing throughout. Emergency medications and airway equipment are on hand at all times.

Dr. Mindi holds an Arizona Oral Conscious Sedation permit, and our clinical team maintains current BLS and PALS certification with regular training in pediatric emergency response.

You're welcome to stay with your child while the medication takes effect.

Afterward

Your child will be groggy, unsteady, and not quite themselves for the rest of the day. Expect it — it passes.

  • An adult needs to watch them all day. Not alone, not with an older sibling. Balance and judgment come back more slowly than alertness does, so a child who seems fine can still be wobbly.
  • Clear liquids first, then soft foods once fluids are staying down.
  • Keep it quiet. No stairs unsupervised, no bikes, no trampolines, no swimming.
  • Mild nausea is common. Small sips, slowly.
  • Most children are back to normal by the next morning.

Call us right away if

  • Your child is very difficult to wake, or won't stay awake
  • Breathing looks noisy, labored, or slow
  • Vomiting won't stop, or they can't keep fluids down
  • There's a fever, or bleeding that doesn't settle
  • Something simply feels wrong to you — you know your child best

Still deciding?

Every parent has questions before agreeing to sedation, and they should. Bring all of them to the consultation. We'd far rather spend an extra twenty minutes talking it through than have you sitting in our waiting room worrying.
