Don't see your plan? We probably still take it.

Not seeing your insurance here does not mean we don't accept it.

There are a genuinely enormous number of dental plans, and new ones appear all the time. No office can list them all, and any list goes out of date the moment an employer switches carriers.

Please just call us. Have your card handy and we'll check your specific plan in a few minutes. Families are often surprised — both by finding we're in-network after all, and by discovering their out-of-network benefit is better than they'd assumed.

We'd hate for a child to go without care because of a list on a webpage.

Plans we don't accept

We are not in-network with HMO or DMO dental plans — these plans that assign you to a specific office and require referrals within a set network. If that's your coverage, you're welcome to be seen with us on a self-pay basis; just know your plan won't contribute.

How we plan your child's treatment

One thing worth saying plainly, because it shapes everything else on this page.

We don't build treatment plans around what insurance pays for.

Every child gets a plan based on what that child actually needs — their exams, their cleaning schedule, their x-rays, their treatment. Some children need more frequent visits than others.

Some need protection that others don't. That's a clinical judgment, and an insurance company's coverage table plays no part in making it.

So if we recommend something your plan doesn't cover, that isn't a sign it's optional. It means we believe your child needs it. Dental benefits are a contract between your employer and an insurance company — they were never designed around your individual child. We have no control over what they cover, what they pay, cost, and more. It is a family’s obligation to know their benefits inside and out. We verify as a courtesy not a determination of treatment

We'll always tell you what the ESTIMATED costs before we do it, and you always decide. But we won't quietly leave a recommendation out because a plan wouldn't reimburse it.

Insurance & financial policy

Your plan decides what it covers. Insurance companies determine your eligibility, your benefit levels, and what fees they consider acceptable. We aren't party to that contract and we can't change their decisions — though we'll appeal on your behalf when we think they've got it wrong

Insurance rarely covers 100% outside of preventative care. Most plans cover somewhere in the range of 50–80% of the total fee depending on the type of treatment, with preventive care usually covered best and major treatment least. There's also an annual maximum — a ceiling on what your plan pays each year, after which costs are yours.

We file your claims electronically for you, in-network or out.

Everything we quote is an estimate. We verify your benefits before treatment and give you our best projection of your portion. But what an insurance company tells us during verification is not a guarantee of payment, and plans sometimes pay differently than they quoted.

Knowing your benefits is ultimately the family's responsibility. We'll help however we can — verifying coverage, filing claims, submitting pre-treatment estimates for larger treatment — but your plan is your contract, and we can't compel an insurer to pay.

If your insurance declines to pay, or pays less than estimated, the remaining balance is the family's responsibility, due within 30 days of the claim being processed.

If your plan pays more than expected and you've overpaid, we'll refund you.

None of this is meant to sound unfriendly — it's here so nothing comes as a surprise. And if a balance ever does surprise you, call us. We'd far rather work it out together than have it become a problem.

Covered by two plans?

If your child is on both parents' insurance, both plans can often contribute. Bring both cards.

One quirk that surprises nearly everyone: which plan pays first is usually decided by whose birthday falls earlier in the calendar year — not by who earns more or whose coverage looks better. If you have a PPO plan and AHCCCS - we are required to use your PPO plan first and submit the remainder to AHCCS. If you fail to provide two insurance plans prior to the visit, you may have to pay out of pocket. Be sure you update us on any changes at least 48hrs in advance.

No dental insurance?

Plenty of our families don't have it, and it shouldn't stop your child from being seen.

We have a wonderful Bloom Membership Plan that provides a fair market cash price value for your child’s two visits per year. Any treatment/procedures not covered in the membership will receive a 20% discount.

Getting the most from your benefits

Black circle with a white checkmark icon

Use your preventive visits. They're usually the best-covered thing in your plan, and they're what prevents the expensive treatment later.

Watch the calendar. Most benefits reset annually and unused amounts don't carry over.

Ask about splitting larger treatment across two benefit years when it isn't urgent. We do this routinely.

Common questions

  • Do you take my insurance?

Call us with your card. The lists above are carriers we're in-network with, but plans within a carrier differ and no list is ever complete.
  • Do you take AHCCCS?

Some AHCCCS plans, not all. Call and we'll check yours.
  • Do you take HMO/DMO plans?

No. You're welcome to be seen on a self-pay basis.
  • Will you see us if we're out of network?

Yes. We'll file your claims and give you an honest estimate first.
  • Why did my plan pay less than you estimated?

It happens, usually because of a plan clause that didn't surface during verification. Call us — we'll go through the explanation of benefits with you and appeal it if it looks wrong. However, what we are able to always provide is an estimate not a guarantee. You may have out of pocket expenses even if they aren't predicted
  • My child needs a lot of work. Can we spread it out?

It depends on what your child needs, how we are going to provide treatment and how far along the cavities are. We do not do payment plans but do accept CareCredit

Ask us before you worry about it

If cost is a concern, tell us. It's a completely normal thing to raise, and we'd far rather plan around it with you than have a child go without care because a family felt awkward asking.
